How do u cure the pains in the ass?! I had 3 job injured over 13 years and carried the PNS (Permanent and Stationary) pains over my lumbar' abdomen and groins of my body. After my worker com case was settled unfairly, I couldn't get anymore treatment  (because the doctors decided it's incurable, and the proved to be good option: Acupuncture and Chiaproctor are not covered in American Medical System, which is the main reason I'm posting and constructing this class action: (Got pains too? the first injured (1994) Angie L forced me to delivery a very heavy LD auto-changer ALONE, it's supposed tobe delivery by 2 or 3 people, but the drivers and helpers were out, I was the only salesman owned a minivan ($20,000 value since brand new, 7 years later it's ruined for working for Sing Young  / Karaoke Music World , but Angie was only mad because I couldn't make the party job for her, said NOTHING about to get the minivan fixed!), Angie gave me a boss-face and demand me to do the delivery, Pedro (the driver) used to do it alone when he got no helpers, u are younger than him, u shall be able to do it!" So 3 people loaded the machine into my minivan, all I had to do was to drive there and move it out load alone. I couldn't left the machine and broke my shoulder and arm. 6 months later, my right arm couldn't move,  Angie L sent me for worker compensation (because she had no medical insurance for employees), and I only got 2 weeks physical therapy. Exactly, only 6 session of cold and hot massages in 2 weeks. my spine never got treatment,  my right arm was able to move, but still hurt and soul. I didn't get any settlement CASH from worker com case?! 13 years later I got to file my own worker com, my attorney told me:"It's STRANGE that u did not receive any cash settlement for your first time injured!" It's not strange, I was renting and living in Angie L's warehouse, all mails went to her office, and they had stolen 3 of my checks (amount$500' $1000 and $1800, all  pay to my name but addressed to her warehouse) because they believed that I worked for her, they were entitled to cash all of my other incomes from other company) 2. the second time injured (2000), my injured lumbar deteriorated into my abdomen and groins, she told me 'Sing Young  / Karaoke Music World  is small business, no medical insurance for worker'. 3. the third time injured (2002), I injured my neck' shoulder and arm while I was operated 3 machines ALONE to videotaping her singing contest, she told me 'Jerome, u are 50 years old, all 50 years old get shoulder pains!' then she showed me her ass and told me that she had pains in her ass. Telling me that I'm old, and showing me her pain in her ass, both didn't cure my pains. 4. Angie L rather to pay the attorney to shut me up, she called to tell me that whatever she did are all over 1 year that I can not sue her, then she hired the attorney Chato to sue me to move out of her warehouse, then she lost the case, because it's in business zone  that she shouldn't rent it to me in the beginning. Angie was tricky enough to get one of our friend from Japan to call me, I was too stupid enough to agree that I would settle all of my lifetime damage she caused  for $10,000 dollars. Angie didn't want to pay it, Attorney CHOATE faxed me a paper for me to sign, that she would pay $5,000 for me to move out,  and I need to pay her attorney fees in the other $5,000. I didn't agree to sign, I want the $10,000.  So Angie L sold the warehouse with me still living in it, she hide the mail of next court, I missed the next court, so after 2 attorneys and 2 courts, she cheated to kicked me out of her warehouse by pay me NOTHING. But I had 2 mistake from the beginning: 1.I told doctors that I have LUMPS on my abdomen (and the LUMPS were break into small lumps)  and every doctor keep telling me they can't feel the LUMPS??? 2.I told doctors that my right testicle hurt and couldn't drop down, and different doctors told me the same thing: they can't find anything wrong with my testicle. Later I had a big muscle LUMP swelling beneath right testicle, and HMO even refused to see me without appointment, I couldn't wait to see emergence department, so I squeezed out some blood out of the swelling lump, Monday I see Dr.Beeve, he gave me some medicine and my right testicle dropped down normally, then I start to figure out the true feeling, my pains are at GROIN! not testicle. I wasted lots of exam' seeing different department because I thought the pains come from testicle. The pains are actually spreading between my abdomen and groin, but didn't get much treatment because all the exam couldn't find the LUMPS and couldn't find wrong in testicle. All the physical treatment refused to put their stimulator' Hyper wave on my abdomen and testicle, they all say it's prohibited area to have these kind machine over there. Also not to mention this to any others or else I will lose my psychiatrist license. What he did was cross my legs on his shoulders and he twist my spine very carefully. Then I heard a soundless popping noise in my lumbar, I felt something was UNLOCKED in that popping sound in my bone. Before that popping in my lumbar, I got all kind of lumbar problem: my spine hurt, my backbone never be straight' my back hurt' my legs got spasm at every cold morning' I couldn't jump, I couldn't run. Right after that popping unlocked something, I felt some magically change to everything link to my lumbar. I felt MUCH MUCH better, I can run, I can jump... everything seemed cured healthy and come back. Psychiatrist told me that my shoulders are not even, my lumbar was hurt and nobody dare to straight it up, so he did it. I felt almost cured, so next week I asked him to do this special adjustment skill again (and this skill have been perform in all the Asian street, those chiropractors without license), but next month I got surprised laid-off by AHT, couldn't afford to see any doctor. All jobs done ALONE. Sing Young Music refused to cover my medical again, she said "People over 50  years old always got shoulder pains!" 10/8/2002, Attorney Jacobson (213-383-0500) arranged me to get totally medical treatment at Metropolitan Medical Group. I get to take 3 kinds of treatment (chiropractor' therapy and acupuncture) at same day! But I always getting the problem of they "only treat 2 spots only"! I have more than 8 aching spots: neck' shoulder' abdomen' groin and lumbar. Also they only want to treat my 3rd time injured only (neck and shoulder), they don't want to hear me say about my 1st and 2nd injured (lumbar' abdomen and groin). I had to beg or fight to make them take turn to treat every spot, each of my aching spot could only get treatment once every week. acupuncture able to put as much needles as I need, but it can only treat one side at one time. So either my front or my back lumbar is getting treatment twice each week. So it take whole year to get good cured. I told the chiropractors about the "unlicensed special Asian adjustment skill" I got before, so the chiropractors start to ADJUST my neck and lumbar too. Dr.Rodney, he agreed, told me "That kind of treatment only cure 70% of pains". which is true. My neck got UNLOCKED and cured, but  my lumbar still slow on progressing cure. later I was able to find a good heart attorney to help me file my own worker com case. Still, I couldn't get justice and good settlement for my ruined life. I only got $8,000 settlement cash, and all of my pains couldn't get any more treatment because they decided it's incurable. His pain KILLER made my oral cavity galled and all swelling, when I went back, they told me I wouldn't get to see the same doctor because the doctor came there as volunteer, so I had to got up at 5 am again, and wait in lines for 2 hours to see second doctor, the second doctor didn't even check or look, he just hear me and gave me another pain KILLER. Did I trust the second doctor to test his second kind of pain killer? No! I didn't throw away the KILLER either.  I heard some news about some wise men try to make new laws, to make American Medical System recognize the Asian style treatment become the official medical treatment method.  Please submit my case and my story to those wise men, I need acupunctures like all the Chinese and Asian people do in other country, no [3 spots only] policy, 7 times each weeks! not [3 sessions of 20 minutes acupunctures each week]! Thursday, 3/13/2008, they put me on treadmill for test, because those PNS pains in my lumbar' abdomen and groins since year 1994, I have not ever run over years,, they stopped me when the machine starting to going faster to get me running, and decided that nothing wrong with my heart. A lady told me that since they did not find me have AMNESIA, so she could not apply medi-care (or medicar ??) for me. The doctor treated and cured my right arm and neck, my groin and trunks got treatment by Asian acupuncture, acupuncture needles able to get into my flank and groin to treat those pains for no problem, but the doctor told me that American medial system doesn't count Asian acupuncture as an officially medical treatment ,  they decided that the acupuncture can never cure 100% of pains, always only 80% of the pains. 10 months later I was released as PNS, the left-over, the 20% uncured my pains were incurable, but wouldn't get worse, also I should be able to work again, under some limit conditions. Two of the judgments are all wrong, and create more problem to the rest of my life. 4/19/2006. Farmer insurance paid me LUMP SUM SETTLEMENT of $7,500 (It's the net total after the attorney fees). 5. My pains got worse, the 20% left-over become pinching pains and PINCHING me every second, I couldn't sit straight over 20 minutes, always exhaust by the consisting pains over my lumbar' flank and groin, always falling into nodding when I was staring at the computer monitor when I was working in office. I got fired and filed for disable insurance at 2004, Department of Social Services denied my application because according to those genius doctors report, Year 2000 no expertise doctors were able to find the reason and cause of my pains. Year 2007, I was struggling and surviving by food stamp and general relief, Social worker helped me to file for disable benefit again, when I finally got another part time job (2 and half days each week) for 1 or 2 months, u denied my disable application, I don't know the reason why u denied my disable this time, but I didn't bother to ask why, until I lost my job after 3 month trial period, my pains still make me have bad office behavior and got me fired, this time, it's shorter, I lost job in 4 months. and this time it's not even a full time job, it's part time job. I need  to sue Farmer Insurance, when Angie filed my worker com case in 1994, Farmer Insurance wouldn't allow me to see my own doctor, they sent me to NSC therapy (I will give u the correct name and address if u care to help), they asked me what's wrong, I said my back hurt all over, and my arms couldn't move. He said, we don't treat whole body, u must tell me exactly which spot hurt, and u only tell NOT MORE THAN 3 spots! Fine! then I said my arm and shoulder can not move, and this spot, that spot and this spots hurt! so they only treat those 3 spots, after 2 weeks my arm was able to move, then they called me CURED and released me, I said 'but my lumbar still hurt!' He said 'we only treat the spots u reported, u did not report your lumbar!' My lumbar never got decent cure, then it got worse to my abdomen, then my groins, now it's my penis and black toes. So, it's the last straw, DMV informed me that they suspended my car registration (10/22/2008) because I don't have money to buy a driver insurance.

I told them that I'm on processing of my disable hearing court, I told them that my disable made me NEGATIVE income to survive, but they suspended my registration anyway, they want me to buy car insurance, they also want me to pay extra $14 to reinstatement my car registration.

I got 3 times job injured at year 1994' 2000 and 2002. After 10 months of physical treatment at 2003, they released me and claimed that my pains were 80% cured, my worker compensation case got settled at 2004, based on 2 of genius doctor's totally wrong decisions:

1. that my left-over pains were PNS, permanent and stationary,  meaning it's incurable, (WHICH IS WRONG!) but it won't get worse (WHICH IS WRONG!! again.).

2. I should be able to work again, under some restriction. (WHICH IS, WRONG!!! again and again.)

Genius, if your are the person in pains, do u still actually believe there are pains is incurable, and won't get worse?!

I got only $7,500 settlement cash, it run out really soon, and I got no more medical insurance and physical therapy for my pains ever since. My attorney advised me to take the settlement, he said if I said  NO, it would take another ONE YEAR to see the hearing court. Also, I could apply for DISABLE after the worker com case.

How could the pains are incurable and won't get worse?!  My 20% PNS pains deteriorated into 30%, 40%, 50%, 60%. pinching me front and back every second, every minute.

I got fired for bad behaviors from the full time office jobs because my pains in my lumbar' groins and flanks made me unable to sit straight over 10 or 20 minutes, I could not concentrated, always fallen into uncontrollable nodding in front of pc monitor, I changed chair, changed seat, changed postures, people always found me either looks drowsy' nauseated, or disappear and stay in bathroom a lots (I have all kind of problem with toiled bowl).

After I lost my job again and again, all of my application got denied, EDD or Social Security, those bureaucracy offices denied me for the reason that I did not file the  disability within 49 days, too old  to get Medi-Cal, too young to get retired, too poor (not enough income) to get benefit, I was on food stamp over months, then had been helped to apply for Disability again.

September 2007, Social Security denied my Disability benefit, AGAIN, I didn't really care, because I was lucky enough to finally find a part time job, I never tell the new boss that I have 3 times job injured and 13 years old pains, I never tell the new boss that I was fired for the pains made me have bad behaviors. The problem is that later my job (TV news translator, I could not finish translating 7 pieces of news to meet their deadline hour most of the time) got cancelled at 2008, I still not tell them my PNS pains problem, they put me in another job, operator in control room, part time job again, 2 evening each week, I was alone after 7 pm, the boss talked to me when they found me leaving at 3 am, I agreed to punching out at 1 am, I still don't want to let them know that I have to spend 10 or more hours to finish the 8 hours job.

March 9-2008,  I was attacked by Disorientation. The city of Pasadena Paramedics,  sent me from 465 N Halstead, to Arcadia, Methodist Hospital on ALS Emergency. According to the 24 hours staff report, I was 'awake and CONFUSED', kept saying I could not find my car. (I was so lucky, it did not attack me when I was driving.) I was talking and not listening, I kept asking 'what's this place?' (then they knew something wrong, because everybody knew that I was a regular member in fitness center), then I would said I can not find my car, then I would ask where am I. They plunked my finger to get my blood for test, they use the lights on my pupil, but I didn't have any response to these things, the scary part is that I don't remember any of these at all.

3-9-08 to 3-13-08, Methodist Hospital kept me hospitalized' monitor watching and test everything for 4 nights. (3/20/2008 Account: 3/9-3/13 Post Icu $7,500.  Cardiology $2,900. Mri -Brain $2,800. Laboratory $2,692. Ct Scan/head $1,800. Peri Vascul Lab $1,250. Stress Test $1,100. Emerg Room $850. Pharmacy $629.94. Eeg $600. Ekg/ecg $300. Dx X-ray/chest $280. Med-sur Supply $24. Sterile Supply $3, and the most horrible attack over all is the TOTAL CHARGES $22,728.99 )

3-10-08, the second night, they transfer me into a solo room for 'confused people', one nurse run into my room and said "Your heart rate is too low, we think it's the battery, I need to change a new battery,"  but after the transmitter got a brand new battery, another  tech people came again, check the battery and the transmitter connected to my heart. Doctor Scot Liang told me "We check your brain wave, nothing wrong, but we find your heart rate is too low." I asked "How low is too low?" He said "The athlete got low heart rate, but your heart rate is lower than the athletes."

The midnight a nurse waked me up and told me that she need to take my heart rate, it seemed that they think my heart was so low that they don't think the transmitter and monitor got  the correct heart rate reading, that I might die on sleeping.

I asked "exactly, what is my heart rate?" I didn't have paper and pen to write, but the nurse told me a number is so easy to remember, she said "You are 44 over 100 something. "The Hospital showed me a pace maker, I refused to be install one into my chest and heart.

3-13-08, after the treadmill test, they decided that  my low heart rate didn't really hurt my health. Doctor Scot Liang released me and told me that he still can not decide what caused my "Disorientation" (He actually NEVER told me about what happened to me during 3-9-08 to 3-13-08, he only told me that he thought that I had AMNESIA but it  turned out that I didn't have any.)

I said "In the first day, I told you that I have 13 years old pains in my lumbar' groins and flank, you said it had nothing to do with why I'm here. NOW, could you hear me one more time, I HAVE PAINS pinching in my lumbar' groins and flank, over 13 years!

So doctor Liang wrote 'follow-up with county clinics for chronic lumbar back pain' on my released form.

5-19-2006, After a hearing court, 5-19-2008, State of California- Health and Human Services agency. Department of Social Services, ordered that Los Angeles shall  rescind the action to deny my Medi-Cal benefit. also ordered that Los Angeles shall refer my application to the DDSD for a determination of disability of the alleged new or different medical impairments.

8/22/08, You denied my general relieve cash, only give me food stamp $75 for 8/2008, and $123 for 9/2008.

I'm only making $664.96 (reported gross income (actual), you only counted my rent (including utility) $470, (I always owe the landlord the rent, my rent is always late, never paid-off), not counting the gas fee is always around $70 to $140, basically I'm living on 3 credit card, and now all 3 credit cards are all out of credit limit, and charging me over-limit fees' overdraft-fee. (3 of my credit cards are un-chargeable with interest fees and charges, adding total debt $3,000. My checking account is always minus balance because of those circulating over-draft and over-lime charges.)

Hospitals and the city of Pasadena ambulance now are sending me to debt collectors because I can never pay the Hospital and ambulance fees of total $25,000 dollars for my 3/9/08 to 3/13/08 disorientation hospitalized charges. Please tell those Hospital to transfer those charges into my medi-cal account. They just treated my mental disorientation last March, and now their debt-collecting letters will send me back into their hospital again.

I'm still waiting for these helps never come in time. Please grant my general relieve cash and whole food stamp, so I can survive to see my Disable application come through.

One disaster come after another, now DMV sent me the notice of intent to suspend my vehicle registration effective at 10/22/2008, because they found out that I'm driving without driver insurance. Also the registration fee will cost me $77, SMOG certification required shall cost me  additional $80 or more for Smog check.

Could u pay these Driver insurance' car registration and smog check fees, so I get to survive to see my Disable application come through. I'm living in the fear of Disorientation may attack me again anytime, Even it's a lousy income of  $664.96 each month, I still need my car for this 2 evenings part time job.

When u are checking this job, please be very aware about this: My boss still never find out that I'm apply for Disable. I never tell them that I have permanent and stationary pains (incurable) in my lumbar' abdomen and groins, It's only me alone in the office after 7 PM, if they found out that I was disorientated last 3/9/08 for mental kind of problem, they will surely fire me, Last 3/9/08 when I was hospitalized, I did not tell them the true, I just told them that I was check-up and the Hospital found out that my heart rate was too low so I have to stay for more exam. After out of hospital, now I'm sense of that aside of my pains in lumbar' abdomen and groins, I do have headache hang-up inside my forehead and temples, I don't do well in my job, they found mistake  I do in my 2 evening job, (my job is to edit the TV program and TV commercial, and they always find out I put some error in TV broadcasting, I know the only reason they still not fire me is because they know if they fire me, they can not find another guy to fill this part time job only working 2 evening each week on $600 paycheck each month, the other guy has 5 evenings and fulltime job.) This is my only income, if u want to call them and tell them what's real happen to me now, please be sure that u can grant me more and more general relieve cash so I can survive to see my Disable application go through. Last week I broadcasted the Taiwanese's commercial in air by mistake. (I remembered I cut it out, but it still aired in the broadcast.) This week I broadcasted yesterday's news by mistake. 6/17/08, Roybal Comprehensive Health center found the tubular adenoma and colonic polyp in my colonoscopy, after the exam, they decided it's not any cancer. It's supposed to be good news, but I'm still here with the same problems (that I still have blood in dung, that my groins still pinching in pains), and  worse.   I'm living in fear of that the next disorientation is striking soon..

Dr.Naing has also refer me to see a specialist to treat my hemorrhoid.

The doctor and the nurse both reminded me the same thing, they ant me to wait patiently, because, Sometimes IT TAKES MONTHS.

My lumbar still hurts, my groins and flanks hurt, and it takes MONTHS to see the specialists.

and all you do is keep mailing me tons of form to fill?! My brains hurt, I don't want to count that how many times I had fill this kind of form, u shall know how many time u had deny my application over those forms I filled!

My soul had already said NO to this world, when my lumbar hurt  and I feel my headache hangover in my temples and foreheads, I know I'm going to get disorientation attack soon.

July 21, 08, I got my results for selected grid cell, summary printed July 21, 08. LAC+USC Medical Center, Date Dict: 03/31/2008. Findings: Lumbar lordosis 脊柱前凸 is maintained. Small anterior osteophytes  of the L3 to L5 vertebral 椎骨 bodies are seen. The vertebral bodies and disc space heights are preserved. The posterior elements are intact. No prevertebral soft tissue mass is seen. The sacroiliac joints are unremarkable. The osseous structures are well-mineralized. The bowel gas pattern is non-obstructives. IMPRESSION: Minimal degenerative disc disease of the lumbar spine as described above.

I'm in pains and waiting for 2 specilists for my bleeding hemorrhoid and Disc bulges, and all I get is next appointment before Christmas?! My lumbar' abdomen' groins' hemorrhoid and spine disc bulges are pinching every second, the hospitals are sending me to the debt collectors for I can never pay the disorientation hospital bill. 1994, I was forced to delivery and move a heavy laser disc auto-changer ALONE,  my spine was damaged and my right shoulder twisted, months later my right arm couldn't move or raised, I was assigned to see DSC Physical Therapy Inc. 410 Arden Ave. #102 Glendale 91203, 818-240-0803 , the genius therapy (actually it's Farmer Insurance assigned doctor, Please grant my disable case, then I will surely go back to sue those genius who destroy my life) wouldn't let me say that my back were all hurting over, he only allow me to tell him NO MORE THAN THREE SPOTS hurting, I said my right arm couldn't even move, and three most hurting spot were this that and that, they treated three spots for 6 sections (half hour), then released me after 2 weeks, released me CURED because my right arm and shoulder was able to move and raised again, I said but my back still hurt, they said we only do the pain spots by book, u did not report the back pains.

3. Year 2000, using abdomen' flanks to move heavy machine, my pains deteriorated into my groin and both my trunks, Sing Young Music World told me that they r small business (actually she have 3 branches) no medical insurance for me. I found a daytime office job, but still living and renting Sing Young's warehouse and working for their DJ job in weekend and nighttime. I use my daytime job's medical coverage to seek help. But no therapy want to use their machines on my groin and abdomen, they said it's organs and testicle inside shouldn't be touched by supersonic or heater.

3/1/2000 to 11/16/2001,  Dr. Gregory Beeve / Verdugo Hills Medical Group sent me to different specialist for exam, then found nothing they could cure,  then gave me all kind of pain killers pills.  (on other 2 pages, enclosed is the list of those PAIN KILLERS, I'm repeating this 13 years old story because I want to sue the American conventional medical system about how they can never cure my kind of pains and eventual ruin my life. Also, last time I applied for disable, your office rejected it using this reason, according to the report of year 2000, no doctors could ever find the reason of my pain, so I can not apply to be disable at year 2005.) Until finally I said that there was one medicine worked to release my pain, and the doctor said ah Ha, u need to see a psychiatrist! because that is not a pain killer, that was a pill to make u relax your emotion.

I would do anything to cure my pains so I paid more money to see the psychiatrist. And here is the difference between these specialists and the psychiatrist: The specialist only saw me 20 minutes and exam me with all kind of machine, the psychiatrist was watching and observing me over month, 3 hours each week. After 1 month, he analysis and cure my nightmare, the next month, he said, Jerome, your right shoulder is lower than your left shoulder, everybody can see what your problem is. (well, not anybody, not those genius specialist ever.) I'm going to perform a physical treatment on u, because u r a Asian, u shall know what kind of physical it is, but I need u to do 2 thing, number 1, u need to agree to let me treat it, number 2, u need to agree NOT to report it, or else I will lose my psychiatrist license. My license is not allow me to do any physical therapy.

What he did was a very common Asian chiapracter do, he put my legs on his shoulder and he twist my legs and I heard a poping noise in my spine, all of sudden I felt my pains locking in my spine were all UNLOCKED, before that noise, I was walking like an old man because that spot was hurting and locking, but he unlocked that spot, he told me that my nerves were CLOCKED (again, my English is never good enough to remember exactly the words he said) he twisted my spine and unlocked it. It was September 2001 terror attacks, all business went down, I got a surprise lay off and lost my job and medical insurance to see the psychiatrist again.

4. Year 2002, I hurt my right shoulder and neck on my third job injured. Sing Young Music World told me that I was over 50 years old, and all 50 years old get the shoulder pain, she even showed me her ass and told me that her ass got pains too, which didn't make me feel any better at all.

This time I found an attorney to file my worker com case. 10/8/2002, Attorney Jacobson filed my Worker Compensation case. 

11/7/2002, Dr.Yousif /Metroploitan saw me. I told the Psychiatrist Asian style's treatment story. (Even I promised the Psychiatrist not to report his treatment, but attorney still found his name and gave it to Farmer Insurance.) Metropolian had 2 sessions of American  style treatment, also, 2 sessions of acupuncture each week. and their chiaproctor gave my spine Asian style adjustment too! 9/3/2003  Dr.Rodney released me as P &S, told me "Acupunctures can only cure 80% of the pains." I got fired and filed for disable insurance at 2004, Department of Social Services denied my application because according to those genius doctors report, Year 2000 no expertise doctors were able to find the reason and cause of my pains.

Year 2007, I was struggling and surviving by food stamp and general relief, Social worker helped me to file for disable benefit again, when I finally got another part time job (2 and half days each week) for 1 or 2 months, u denied my disable application, I don't know the reason why u denied my disable this time, but I didn't bother to ask why, until I lost my job after 3 month trial period, my pains still make me have bad office behavior and got me fired, this time, it's shorter, I lost job in 4 months. and this time it's not even a full time job, it's part time job.

What's the point of repeating these 13 year's old story? There is ONE.

Year 2004 (or 2005) I applied for Disability benefit (or SSI ?), there was one Social Security office denied my application, she said "In year 2000, doctors could not  find your pain's problem. so I deny your disability.

Dear lady, you deny my SSI and make me surfer for the rest of my life, every second and every minute.

Here is the genius doctors they could not find the problem and gave me all kind of pain killer to kill me softly and slowly.

It's supposed to be good news, but I'm still here with the same problems (that I still have blood in dung, that my groins still pinching in pains), and  worse.   I'm living in fear of that the next disorientation is striking soon..

Dr.Naing has also refer me to see a specialist to treat my hemorrhoid.

The doctor and the nurse both reminded me the same thing, they ant me to wait patiently, because, Sometimes IT TAKES MONTHS.

My lumbar still hurts, my groins and flanks hurt, and it takes MONTHS to see the specialists.

and all you do is keep mailing me tons of form to fill?! My brains hurt, I don't want to count that how many times I had fill this kind of form, u shall know how many time u had deny my application over those forms I filled!

It will takes another 4 to 6 months, what's the point of repeat this bureaucracy paper works?! When they deny it again, you will order them to rescind it again? But I realize the difference this time, I'm going to get disorientated again, and sooner! Because after the disorientation and hospitalized, now I have a clearer sense that additional to the pains in my lumbar' groins and flank, I also got headache hangover in my forehead' deep inside of my temples and brain.

July 9,2008, DEPARTMENT OF SOCIAL SERVICES, Disability Determination Service Division, (213-736-7118, PO Box 54800, Los Angeles, CA 90054. DDS Case #696539) mailed me MSC 121 form, ask me to have my current treating doctor to fill and return the form within 14 days. 7-23-08 is the deadline? El Monte center is a county facility, I had called over weeks and could not get a appointment for the follow up with my treating doctor, Dr.Wolfgang Rodriguez, finally they gave me an appointment with Dr.Naing at 7-21-08.

7-21-08, Dr.Naing told me that he can not fill this MSC 121 form for me since this is his first time to treat me.

He read Dr.Wolfgang Rodriguez's note to me, they found the disc bulges in my MRI result, so Dr.Wolfgang Rodriguez had already referred me to a spine specialist. (and why I never get any notice about this?!) Roybal Comprehensive Health center found the tubular adenoma and colonic polyp in my colonoscopy, after the exam, they decided it's not any cancer. It's supposed to be good news, I told Dr.Naing that I'm still here with the same problems (that I still have blood in dung, that my groins still pinching in pains), Dr.Naing had also refer me to see a specialist to treat my hemorrhoid.

I got 3 times job injured at year 1994' 2000 and 2002. After 10 months of physical treatment at 2003, they released me and claimed that my pains were 80% cured, my worker compensation case got settled at 2004, based on 2 of genius doctor's totally wrong decisions:

1. that my left-over pains were PNS, permanent and stationary,  meaning it's incurable, (WHICH IS WRONG!) but it won't get worse (WHICH IS WRONG!! again.).

2. I should be able to work again, under some restriction. (WHICH IS, WRONG!!! again and again.)

Genius, if your are the person in pains, do u still actually believe there are pains is incurable, and won't get worse?!

September 2007, Social Security denied my Disability benefit, AGAIN, I didn't really care, because I was lucky enough to finally find a part time job, I never tell the new boss that I have 3 times job injured and 13 years old pains, I never tell the new boss that I was fired for the pains made me have bad behaviors. The problem is that later my job (TV news translator, I could not finish translating 7 pieces of news to meet their deadline hour most of the time) got cancelled at 2008, I still not tell them my PNS pains problem, they put me in another job, operator in control room, part time job again, 2 evening each week, I was alone after 7 pm, the boss talked to me when they found me leaving at 3 am, I agreed to punching out at 1 am, I still don't want to let them know that I have to spend 10 or more hours to finish the 8 hours job.

Then I got disorientated and hospitalized over 3-9-08 to 3-13-08, (Methodist Hospital, File 42428, Los Angeles, CA 90074. 300 West Huntington Dr., Arcadia, CA 91007. Account Representative, Marie H Adame, 626-574-3597. Hospital Account #0012846770, 3/09/08-3/13/08. )

I still not tell my job the true, I told the manager "I was taking a physical exam, they found my heart rate is too low, so I need to stay overnight for more exam and test."

I get pay of $600 each month on this 2 evening each week control room operator job, it's my only income, I need to struggle to survive, better than your food stamp and general relieve, but never enough to pay off my credit cards' debt, you can still mail me tons of application forms to fill, ask me to write about how those genius doctor decided that my pains are incurable and supposed not to be worse, ask me how I was injured in my job, make me wait and struggling forever then deny all of my application benefit.

BUT, don't ever let my only job learn that I was hospitalized for mental situation! It's always only me alone in the office after 7 pm. I will surely lost this only job if they know my hospital records!

March 9-2008,  I had attacked by Disorientation. The city of Pasadena Paramedics,  sent me from 465 N Halstead, to Arcadia, Methodist Hospital on ALS Emergency. According to the 24 hours staff report, I was 'awake and CONFUSED', kept saying I could not find my car. (I was so lucky, it did not attack me when I was driving.) I was talking and not listening, I kept asking 'what's this place?' (then they knew something wrong, because everybody knew that I was a regular member in fitness center), then I would said I can not find my car, then I would ask where am I. They plunked my finger to get my blood for test, they use the lights on my pupil, but I didn't have any response to these things, the scary part is that I don't remember any of these at all.

3-9-08 to 3-13-08, Methodist Hospital kept me hospitalized' monitor watching and test everything for 4 nights. (3/20/2008 Account: 3/9-3/13 Post Icu $7,500.  Cardiology $2,900. Mri -Brain $2,800. Laboratory $2,692. Ct Scan/head $1,800. Peri Vascul Lab $1,250. Stress Test $1,100. Emerg Room $850. Pharmacy $629.94. Eeg $600. Ekg/ecg $300. Dx X-ray/chest $280. Med-sur Supply $24. Sterile Supply $3, and the most horrible attack over all is the TOTAL CHARGES $22,728.99 )

3-10-08, the second night, they transfer me into a solo room for 'confused people', one nurse run into my room and said "Your heart rate is too low, we think it's the battery, I need to change a new battery,"  but after the transmitter got a brand new battery, another  tech people came again, check the battery and the transmitter connected to my heart. Doctor Scot Liang told me "We check your brain wave, nothing wrong, but we find your heart rate is too low." I asked "How low is too low?" He said "The athlete got low heart rate, but your heart rate is lower than the athletes."

The midnight a nurse waked me up and told me that she need to take my heart rate, it seemed that they think my heart was so low that they don't think the transmitter and monitor got  the correct heart rate reading, that I might die on sleeping.

I asked "exactly, what is my heart rate?" I didn't have paper and pen to write, but the nurse told me a number is so easy to remember, she said "You are 44 over 100 something. "The Hospital showed me a pace maker, I refused to be install one into my chest and heart.

3-13-08, after the treadmill test, they decided that  my low heart rate didn't really hurt my health. Doctor Scot Liang released me and told me that he still can not decide what caused my "Disorientation" (He actually NEVER told me about what happened to me during 3-9-08 to 3-13-08, he only told me that he thought that I had AMNESIA but it  turned out that I didn't have any.)

I said "In the first day, I told you that I have 13 years old pains in my lumbar' groins and flank, you said it had nothing to do with why I'm here. NOW, could you hear me one more time, I HAVE PAINS pinching in my lumbar' groins and flank, over 13 years!

So doctor Liang wrote 'follow-up with county clinics for chronic lumbar back pain' on my released form.

6/5/08, they found blood in my stool, so I was arranged to take a colonoscopy at LAC+USC center. I looked at the monitor and asked which number reading my heart rate, they pointed it to me, my heart rate was 54.

6/17/08, I went to Roybal Comprehensive Health center for biopsy result, Dr. Makino? (323-226-7973, supervising by Dr. Kvine? ) told me that they found the tubular adenoma and colonic polyp in my colonoscopy, after the exam, they decided it's not any cancer. If they didn't take it out, it can develop into cancer within 10 years, doctor want me to take another colonoscopy to make sure the adenoma and polyp is not growing back.

It's supposed to be good news, but I'm still here with the same problems (that I still have blood in dung, that my groins still pinching in pains), and  worse. (after the tests, I'm supposed to go back to Doctor Rodriguez so he can really treat my pains, but I called the El Monte center over weeks, I could not get the appointment to see Dr.Rodriguez, El Monte center is a clinic for patients without medical insurance like me, Dr.Rodriguez is always booked for the next week and the operator was always want me to try to call next week.)

and my hangover headaches grow in my temples and forehead,  I'm living in fear of that the next disorientation is striking soon..

So you want to know how is the relationship I got with other 3rd party? I'm divorced. I share a room with a Chinese couple, they don't speak much English, sometimes we are fine. The rent is $470, but I'm only making $600 each month, so my rents are always late every month, basically I'm surviving on 3 credit cards, and 3 credit cards are all over limits  now. Last time when my rent was 2 months late, she want me to move out, when she found out that I was disorientated and not coming home over 5 days, she want me to move out immediately because she said it would be a disaster if next time I'm disorientated in my room and with the door lock locked. But after I gave her my rent, we are talking  with smiles again. I'm working 2 evening 5 pm to 1 am, and I always spend 10 hours to finish my job, always leave at 3 am, still get the pay for 8 hours, (2 evening each week)  I never let my boss know that I was out and hospitalized over 5 days. I'm always alone in the office after 7 pm, if they find out that I had so-called mental hospital record, they will fire me for sure, and it's my only income, even it's a $600 part time job. I chose not to write your forms about my job, do me a huge favor, you only allow to contact my job when you grant any benefit. Over these years, what u did was to make me submit the forms about my job' and what those genius doctors said about my health, then u denied my application. 'm so nice and gentle to myself, never give myself any pressure, my hangover headache already told me that my soul want to leave all the time, please don't do anything harmful to my soul and my mind.

This time u do what u want to do, just don't ever make me lose this only job. THANX.

I'm really  exhausted. My lumbar hurt,  my groins hurt, my flank hurt, then I realized my forehead and brain are blinking hurt all the time.

Your forms make me even sicker and sicker more. I always fall to nodding when I try to concentrating on anything. Please send  help if u really want a complete form so u can deny me more officially. I play your game  all of my miserable life already, send me a help to finish this repeating game this time. Before my soul run away again.

7-19-2008, last Saturday, my lumbar was really hurt, cutting more than the regular pinching, I was in the 24 hour fitness center class, the instructor asked everybody put their finger on neck to take their own heart beat counts, so I counted my heart beats too, after 10 seconds (or, maybe 20 seconds? I'm a little bit SLOW to figure out these detail, also, thinking hurt my brain more.) the instructor told the class to stop and time the heart beat count number by 10, she said: "You are supposed to be around 160 or 170, if you are not nine years old, and you are over 190, you better talk to me!"

The whole class laughed and think it's funny.

I was frustrated and a little bit frighten, because I got 11 or 12 counts, so my heart beats was 110 or 120 when the class were 160? and 190 is a problem if you are not 9 years old?!

Then I remembered how I got disorientated last time at 3-9-2008, (I stayed in hot water shower over hour, then I got absent minded, then I lost myself, found myself hospitalized over 5 days.)

This time I kept myself highly alerted. Yes, all the feeling match my last time disorientated experience, addition to the daily tortures of pains in my lumbar' groins and flank, it is the LOW heart rates make me drowsy, absent minded, and eventually disorientated.

This time I made myself (especially my soul) stay in clear mind, wouldn't allow myself to be drown in the hot water shower, wouldn't allow my mind to be escaped, and make me come home safely, and finish this note.

3/13/2008 I was in Hospital, and the Hospital kindly to call [Green Valley Cab] for me.When I asked the driver if they take credit card, the female driver told me the minimum charge is $25, so I gave her my HSBC credit card number. then she read it to the office over phone.

I was stayed in Hospital of a sudden accident, I didn't have much cash. When the cab delivery me home from Hospital to the place, I found the charge was only $12.50 (or maybe even cheaper ?? I don't quiet remember the exactly amount, could be $11 dollars something? I remembered that I was holding $3 in hand prepare to pay her as tips. When I found out that I did have enough cash to pay, I told her to cancel the credit card charge because I found out that I did have enough cash to pay, I gave her $3 dollars cash plus two more of $5 bills, and she satirized and told me "One dollar twenty cents TIPS???", when I remind her to cancel the credit card charge, her response and expression have made me realized what's in her mind already. When I get online to check, of course she did and of course the Green Valley Cab did! They double charged me! I paid it in cash, and they still charged me in $25, so  called 'Minimum charge'!

Dear HSBC, please definitely cancel this evil charge, also please DO write them a letter and let them know how I feel! I was in a medical accident and Hospital called this cab, and they are doing this to the patient of Hospital?!

This is really evil to do this to the patient, she though that I was still dizzy from hospital ? or she was mean because I could only paid her one dollar something tips?! or because she though I'm an Asian people?  I speak English!

Please DO forward my letter to the Green Valley Cab, she thought that I was a weak patient that I couldn't never remember the cab name and address.

In fact, I'm think suing them if I don't receive their response in one week!! It's already sharking enough that I had to stay in Hospital, and it's even more damages after what this Green Valley Cab do !

ALS 的起因

While I'm waiting for your notification letter about my state hearing for my disable case, I got a medical accident attacked. Something maybe like 'Disorientation'. I stay in 24 hours fitness center shower a lot, since I can not get any medical treatment, I always  use shower room's strong water to treat my pains of lumbar' flank and groin.

March 9th, I showered at fitness Gyms in Pasadena around 2 pm, then I found out that I WAKED UP in Methodist Hospital in Arcadia around midnight.  They told me that when the security guy found me in front of Gyms entry gate next to the parking lot, I was awake and confused, I couldn't answer any question, all Gym's clerks know me, but I couldn't say my name, just kept saying 'I can not find my car' and my car was actually in the parking lots. They plunked my finger to get my blood for test? (it sounds painful but I don't remember.)

The Hospital kept me with  intravenous drip (maybe I cal it wrong in English? it's hanging bag liquid connected to my blood vessel) over nights for test and exam. The  second day (or third day) they couldn't find anything wrong with my brain but they found my heart rate was too low. The other doctor came to show me a pace maker, told me if they found my heart got problem that I may need to plant a pace maker to care my heart.

The doctor said that he couldn't know why my nerves CRASHED DOWN (or some other word he used, my English is never good enough to remember exactly the words he was using). I said how about because my lumbar' flank and groin are in pains every second, my nerves are bearing with with those PNS (permanent and stationary) every second. I told him that 1994 I hurt my right shoulder and spine on the first job injured, but the physical therapist wouldn't let me to say that my back was hurting all over and my right arm couldn't move, I was only allow to report not more than 3 spots of hurting spot, and they only treat those 3 spots, so I said my right arm couldn't move, and it's these 3 spots hurt, 2 weeks latter they released me and said that I was cured  because my right arm could move again, I said my back still hurt, they said u did not report the back pains, we only treat the spots u reported. Year 2000 the pains developed into my trunks and groins, no doctor want to treat it because it's organs and testicle in my pain spots, they just kept send me to different experts for all kind of scan'  test and exam, still, no doctor could ever find out the real  problem and gave me different kind of pain killers, after I ate over 2 pages of pain killers (I still have these 2 pages of pills list) I told the doctor that finally one pain killer working, my pains was somehow better, the genius doctor said 'Ah ha, which mean your pains are from your mind, u need to take my advise, go see a psychiatrist, because that's not a  pain killer, that's only a pill to relax your emotion.

I would do everything to get my pains cured, so I spent more money to see a psychiatrist, I lie  down and talk to him about my life problems, all the doctors watched me for 20 minutes and sent me go through all kind of machine and decided that they couldn't find my pains problem, but this psychiatrist had watching' observing me 2hours each week, he cured my nightmare in the first month, then in the second month, he want me to sit' stand' walk in front of him, then he said 'Jerome, your left shoulder is always lower than the right shoulder, everybody can see your problem, I'm going to perform a special physical therapy on u, because u r an Asian, u should know what kind of treatment I'm doing, but number 1, u need to agree that let me perform this spine adjustment to u, number 2, u should not report this treatment I do, because I will lose my license if u report that I perform physical therapy, I agreed and he put my legs on his shoulder and he twist my legs and I heard a pumping noise in my spine, all of sudden I felt my pains locking in my spine were all UNLOCKED, before that noise, I was walking like an old man because that spot was hurting and locking, but he unlocked that spot, he told me that my nerves were CLOCKED (again, my English is never good enough to remember exactly the words he said) he twisted my spine and unlocked it. It was September 2001 terror attacks, all business went down, I got a surprise lay off and lost my job and medical insurance to see the psychiatrist again. Year 2002, I hurt my right shoulder and neck on my third job injured. This time I found an attorney to file my worker com case. The doctor treated and cured my right arm and neck, my groin and trunks got treatment by Asian acupuncture, acupuncture needles able to get into my flank and groin to treat those pains for no problem, but the doctor told me that American medial system doesn't count Asian acupuncture as an officially medical treatment ,  they decided that the acupuncture can never cure 100% of pains, always only 80% of the pains. 10 month later I was released as PNS, the left-over, the 20% uncured my pains were incurable, but wouldn't get worse, also I should be able to work again, under some limit conditions. So I only got $8,000 settlement money for worker compensation. and all the doctors decisions are all wrong. My pains got worse, the 20% left-over become pinching pains and hurting me every second, I couldn't sit straight over 20 minutes, always exhaust by the consisting pains over my lumbar' flank and groin, always falling into nodding when I was staring at the computer monitor when I was working in office. I got fired and file disable insurance at 2004, Department of Social Services denied my application because according to those genius doctors report, Year 2000 no expertise doctors were able to find the reason and cause of my pains.

Year 2007, I was struggling by food stamp and general relief, Social worker helped me to file for disable benefit again, when I finally got another part time job (2 and half days each week) for 1 or 2 months, u denied my disable application, I don't know the reason why u denied my disable this time, but I didn't bother to ask why until I lost my job, my pains still make me have bad office behavior and got me fire, this time, it's shorter, I lost job in 4 months. and this time it's not even a full time job, it's part time job. I required u to reconsider my disable application again, When I was waiting for my state hearing court, I found myself wake up in the Methodist Hospital.

I was too weak to tell the whole story of my 13 years old pains, the doctor sent me for a MRI scan, and told me that I should give u a copy of Methodist Hospital documents and reports to my disable case.

3/13/2008, the hospital send me to the test on walking and running on their treadmill. and decided that even my hear rates are too low, but it's still nothing wrong about my heart.

The hospital worker told me that because the doctor found me have NO AMNESIA , so she can not apply medicare for me.

The doctor also sign me to go to San Gabriel Valley clinics & health centers for 'follow-up with county clinics for chronic lumbar back pain'. so there shall be more of test result coming soon.

Please help, and help me soon, I'm in worse situation than ever, I still can not keep a job (even it's a 2 and half days each week, part time job), I'm only making $200 dollars for 2 months now, my pains still getting no cure, I can not find a job, even I find a part time job, I lose the job, and now I'm in the fear of I may getting this medical accident attacked. Something maybe I shall call it  'Disorientation' (my friend teach me the word) or' awake and confused' may happen to me again.

I'm living my life like there is no more tomorrow, I got the fear that I may be lose my conscious again anytime. the scary part is not the doctor can not find the reason and the problem, the scary part is that I don't know when it's going to attack me again.

Ever since my worker compensation case was closed, I'm always in out of job and no insurance situation over years. The Hospital released me as PNS (permanent and stationary) meaning, the left-over pains of my lumbar' abdomen and groins are incurable and wouldn't get worse, also I should able to work (under some limited condition),  The hospital report are turn out to be both wrong, my pains are getting worse and become pinching pains after my worker com case was closed, also I got fired twice from my job because of 'bad behavior' (my pains made me can not sit straight over 20 minutes, also the pains cause me drowsy, fallen into uncontrollable nodding while I stare at the monitor.

I may have 2 way to pay your Hospital bill, but they having give me any answer. Maybe if u can help me on this:

1. I requested a state hearing for my disable insurance, Case number: 08056025, (State of California, Department of Social Services. PO. Box 944243, MS 19-37. Sacramento, CA 94244-2430. 1800-743-8525. Case number: 08056025. Appeals & state hearings, 3833 S. Vermont Ave., 4th fl., room 466, Los Angeles, CA 90037. 323-730-6616  Deborah Barmore.) and they mailed me a letter saying that my hearing court will be coming soon last time, but, while I was waiting for their next letter to inform me the court date,  out of blue? all of sudden, I got disoriented attack, and the security called the ambulance to send me to your Methodist Hospital and hospitalized over 3/09 to 3/13, Hope if State of California, Department of Social Services grant my disable insurance, maybe they will pay for the bill of Methodist Hospital for me? Could you tell me if this Disable hearing court will help me? and help me to do so. Before you send me the bills and charges statements, I have mailed your Hospital released records and report to the State of California, Department of Social Services already, maybe I shall mail in your bills and statement? Could you help me to tell them the detail of my 3/9/08 to 3/13/08 Hospital case?

2. After exam and test, Methodist Hospital found nothing wrong about my brain and heart, When Dr. Scott Liang (from Methodist Hospital) told me that he can not find out the reason why my nerves crashed down , I said "How about because my 13 years old PNS pains of my abdomen' groin and lumbar are PINCHING me every second and minutes, over the 13 years already (No more physical treatment after my worker com case was closed in a very lousy settlement because they decide that my pains wouldn't get worse and I should be able to work), Dr. Liang gave me a release form and got me able to go to EL Monte Comprehensive Health Center to get the follow-up of my back pains (I had to explain to the doctor, it's not exactly my back pain, it's my pinching pains in lumbar' abdomen and groin), The Heath Center granted me the OUTPATIENT REDUCED-COST plan, ELIGIBILITY PERIOD is from 3/1/2008 to 2/28/2009 ! I get one year Hospital free of charges.

But, when I showed her your bills from Methodist Hospital, she said that Methodist Hospital is a private Hospital,  the plan won't cover it. I explain to her that 3/9 to 3/13 case was a emergency medical accident, I was disorientated and could not chose to be in community service or private hospital, she just said no.

Is there any way, you can contact the El Monte community Health Center for me? please.

3. If both the disable court and the community plan won't work, PLEASE help me to get a free medical plan to pay your bills please! Encloses is the copy of my one year of free hospital plan, also, my pay check last month, showing I'm making only $168.76, I owe my landlord 2 month rents already, she wants me to move out if I still can not pay the rent next month.

I will surely be there for my Hearing, but I have to mail you my statement before the date, in case of there may be some chance I can not make it myself.

Sunday, 3/9/08, I had attacked by disorientation (I didn't learn this disease name until 2 weeks later I talked to the doctor in El Monte Health center for the follow-up) at March 9th Sunday for no reason, no pre-warning, no symptoms, no signs. The city of Pasadena Paramedics, (100 N. Garfield Ave. #121, Pasadena, CA 91109, 626-744-4179. IV Supplies $50. Supplies $25. ALS Emergency Base Rate $954. Mileage, 5= $17  Oxygen $54.25, Total= 1,168.25) sent me from 465 N Halstead, to Arcadia, Methodist Hospital on ALS Emergency.

I didn't have ALS attacked, According to the 24 hours staff report, I was 'awake and CONFUSED', could not say my name, kept asking where I was, kept saying I could not find my car. (I was so lucky, it did not attack me when I was driving???) They plunked my finger to get my blood for test, they use the lights on my pupil, but I didn't have any response to these things, and don't remember any of these.

Methodist Hospital kept me hospitalized' monitor watching and test everything for 4 nights. (I have to copy those charges and fees, help! I don't even have money to pay the rent over 2 months now! 3/20/2008 Account, 3/9-3/13 Post Icu $7,500.  Cardiology $2,900. Mri -Brain $2,800. Laboratory $2,692. Ct Scan/head $1,800. Peri Vascul Lab $1,250. Stress Test $1,100. Emerg Room $850. Pharmacy $629.94. Eeg $600. Ekg/ecg $300. Dx X-ray/chest $280. Med-sur Supply $24. Sterile Supply $3, and there is the most horrible attack over all, TOTAL CHARGES $22,728.99 )

Monday, Dr. Liang told me that they found nothing wrong (in my Urine? THYROID TESTING ?ANEMIA TESTING ? Drug of abuse - blood? Alcohol ? Immunology: Infectious disease and Complete blood count ?) I told him that I have PNS (permanent and stationary) pains in my Lumbar' abdomen and groins, he said it have nothing to do with why I was in hospital. and he scheduled a MRI test for my pains.

Tuesday, Dr. Liang told me that They found NO WRONG in my brain wave, then they found out that my heart rates were TOO LOW (he said that some athlete's got lower heart rate, but my heart rate is really too low) over those 4 nights, the nurses kept waking me up and measure my heart rate and ask me if I was OK.

Wednesday, Heart Specialist Stephen Soldo Md. came to show me a Pace Maker, and suggest me to install one into my heart.

Thursday, they put me on treadmill for test, because those PNS pains in my lumbar' abdomen and groins since year 1994, I don't ever run over years,, they stopped me when the machine starting to going faster to get me running, and decided that nothing wrong with my heart.

A lady told me that since they did not find me have AMNESIA, so she could not apply medi-care (or medicar ??) for me.

Dr. Liang told me that they can not figure out why I got this into their Hospital. I told him that my PNS pains in my lumbar' abdomen and groins are PINCHING me every second, every minutes since year 1994. He wrote me a release form and get me to call  EL Monte comprehensive Health Center for follow-up for my pains in back (which is actually more than chronic pain in my back, it's in my lumbar' abdomen and groins, I told him the pains came from 3 times job injured 94' 2000 and 2002, over last 13 years, he just wrote it as chronic pains in back).

3/27/2008, I went to El Monte Health Center to take X-rays and Poo test, now I'm waiting to have the test come to the result so Dr. Wolfgang Rodriguez will treat my pains in next month. Dr. Rodiguez said the word then I learn it, that I was 'Disorientated' and Methodist Hospital could not find the reason. 

After released from Methodist Hospital, my pains still pinching me every second and every minutes, I got a TOTAL CHARGES $22,728.99 (plus the ALS Emergency Base Rate $954, IV Supplies from The city of Pasadena Paramedics, total 1,168.25) so far, I believe there will be more charges and bills coming, I was OUT, didn't remember who helped and I believe I will know them all when their bills keep coming. Try to separate from the pinching pains in my lumbar' abdomen and groins, I start to realize that there is a headache HANGOVER in my right forehead, I feel drowsy whole day that I can sleep all day, and still able to go to sleep in the nighttime. I'm in a big fear of this Disorientation will attack me again any sooner, but I try to relax myself and NOT to be fear of it so I can survive to see you in my HEARING DATE:  April 14,2008, 12:30 PM, 811 Wilshire Blv. Room 1118, Los Angeles, CA.

In case if something attack again and u can not see me in the Hearing, here are the summary of my statement:

1. I'm drowsy and having a headache hangover in my right forehead (inside of my right temple), I can sleep whole day and still able to fall into sleep at night, but I will surely survive to see u in the Hearing, if u don't see me there, I must be OUT or whatever again, help me. I share a room with a Chinese couple at 241 San Marcos St. #A, San Gabriel, CA 91776. I don't have any phone, their phone is 626-458-7706 (will go into Fax if nobody pick up within 3 rings.) The pains in my lumbar' abdomen and groin occupied my feeling all the time, if I separate (or try to ignore these 13 years old pains) them and be concentrated on my head, the right temple's headache is heavier than my left temple.

2. My health and my life was ruined by 13 years working for a cheap and evil boss Sing Young Music World, $80 for my DJ job which is supposed to be 2 or 3 people's job, the boss collect $550 or more  for each party, no medical coverage for me.

1994, I was forced to delivery and move a heavy laser disc auto-changer ALONE,  my spine was damaged and my right shoulder twisted, months later my right arm couldn't move or raised, I was assigned to see DSC Physical Therapy Inc. 410 Arden Ave. #102 Glendale 91203, 818-240-0803 , the genius therapy (actually it's Farmer Insurance assigned doctor, Please grant my disable case, then I will surely go back to sue those genius who destroy my life) wouldn't let me say that my back were all hurting over, he only allow me to tell him NO MORE THAN THREE SPOTS hurting, I said my right arm couldn't even move, and three most hurting spot were this that and that, they treated three spots for 6 sections (half hour), then released me after 2 weeks, released me CURED because my right arm and shoulder was able to move and raised again, I said but my back still hurt, they said we only do the pain spots by book, u did not report the back pains.

3. Year 2000, using abdomen' flanks to move heavy machine, my pains deteriorated into my groin and both my trunks, Sing Young Music World told me that they r small business (actually she have 3 branches) no medical insurance for me. I found a daytime office job, but still living and renting Sing Young's warehouse and working for their DJ job in weekend and nighttime. I use my daytime job's medical coverage to seek help. But no therapy want to use their machines on my groin and abdomen, they said it's organs and testicle inside shouldn't be touched by supersonic or heater.

3/1/2000 to 11/16/2001,  Dr. Gregory Beeve / Verdugo Hills Medical Group sent me to different specialist for exam, then found nothing they could cure,  then gave me all kind of pain killers pills.  (on other 2 pages, enclosed is the list of those PAIN KILLERS, I'm repeating this 13 years old story because I want to sue the American conventional medical system about how they can never cure my kind of pains and eventual ruin my life. Also, last time I applied for disable, your office rejected it using this reason, according to the report of year 2000, no doctors could ever find the reason of my pain, so I can not apply to be disable at year 2005.) Until finally I said that there was one medicine worked to release my pain, and the doctor said ah Ha, u need to see a psychiatrist! because that is not a pain killer, that was a pill to make u relax your emotion.

I would do anything to cure my pains so I paid more money to see the psychiatrist. And here is the difference between these specialists and the psychiatrist: The specialist only saw me 20 minutes and exam me with all kind of machine, the psychiatrist was watching and observing me over month, 3 hours each week. After 1 month, he analysis and cure my nightmare, the next month, he said, Jerome, your right shoulder is lower than your left shoulder, everybody can see what your problem is. (well, not anybody, not those genius specialist ever.) I'm going to perform a physical treatment on u, because u r a Asian, u shall know what kind of physical it is, but I need u to do 2 thing, number 1, u need to agree to let me treat it, number 2, u need to agree NOT to report it, or else I will lose my psychiatrist license. My license is not allow me to do any physical therapy.

What he did was a very common Asian chiapracter do, he put my legs on his shoulder and he twist my legs and I heard a poping noise in my spine, all of sudden I felt my pains locking in my spine were all UNLOCKED, before that noise, I was walking like an old man because that spot was hurting and locking, but he unlocked that spot, he told me that my nerves were CLOCKED (again, my English is never good enough to remember exactly the words he said) he twisted my spine and unlocked it. It was September 2001 terror attacks, all business went down, I got a surprise lay off and lost my job and medical insurance to see the psychiatrist again.

4. Year 2002, I hurt my right shoulder and neck on my third job injured. Sing Young Music World told me that I was over 50 years old, and all 50 years old get the shoulder pain, she even showed me her ass and told me that her ass got pains too, which didn't make me feel any better at all.

This time I found an attorney to file my worker com case. 10/8/2002, Attorney Jacobson filed my Worker Compensation case. 

11/7/2002, Dr.Yousif /Metroploitan saw me. I told the Psychiatrist Asian style's treatment story. (Even I promised the Psychiatrist not to report his treatment, but attorney still found his name and gave it to Farmer Insurance.) Metropolian had 2 sessions of American  style treatment, also, 2 sessions of acupuncture each week. and their chiaproctor gave my spine Asian style adjustment too! 9/3/2003  Dr.Rodney released me as P &S, told me "Acupunctures can only cure 80% of the pains." The doctor treated and cured my right arm and neck, my groin and trunks got treatment by Asian acupuncture, acupuncture needles able to get into my flank and groin to treat those pains for no problem, but the doctor told me that American medial system doesn't count Asian acupuncture as an officially medical treatment ,  they decided that the acupuncture can never cure 100% of pains, always only 80% of the pains.

10 months later I was released as PNS, the left-over, the 20% uncured my pains were incurable, but wouldn't get worse, also I should be able to work again, under some limit conditions. Two of the judgments are all wrong, and create more problem to the rest of my life. 4/19/2006. Farmer insurance paid me LUMP SUM SETTLEMENT of $7,500 (It's the net total after the attorney fees).

5. My pains got worse, the 20% left-over become pinching pains and PINCHING me every second, I couldn't sit straight over 20 minutes, always exhaust by the consisting pains over my lumbar' flank and groin, always falling into nodding when I was staring at the computer monitor when I was working in office.

I got fired and filed for disable insurance at 2004, Department of Social Services denied my application because according to those genius doctors report, Year 2000 no expertise doctors were able to find the reason and cause of my pains.

Year 2007, I was struggling and surviving by food stamp and general relief, Social worker helped me to file for disable benefit again, when I finally got another part time job (2 and half days each week) for 1 or 2 months, u denied my disable application, I don't know the reason why u denied my disable this time, but I didn't bother to ask why, until I lost my job after 3 month trial period, my pains still make me have bad office behavior and got me fired, this time, it's shorter, I lost job in 4 months. and this time it's not even a full time job, it's part time job.

Last month I required u to reconsider my disable application again, 3/9/2008, When I was waiting for my state hearing court, I found myself 'wake up' in the Methodist Hospital. Hospitalized and released at 3/13/2008.

My pains in lumbar' groin and flank cause me can not sit straight over 20 minutes, always fall into uncontrollable nodding, make me lost 1 full time job in 2006, I applied for Disable, U denied it because according to year 2000's report, no doctor could ever found the reason why I was in pain. After struggling by food stamp and general relieve money, Social Service applied Disable for me again, u denied again when I found a part time job for trial over 2 months.

Now I lost that part time job, and request my Disable hearing, this time I got Disorientation attacked,  the scary part is that nobody know when it will be attacking again.

The real problem is that I never tell my new boss about my PNS pains problem, and I still lost my full time job, then I lost my part time 3 days job. It's a news editor job, I could not edit 7 news each to meet the production department's deadline in time, so they cancel my job.

This time I don't even dare to tell new boss my Disorientation problem, I'm living in the fear of Disorientation maybe attacking anytime, I'm making only $191.29, $168.76, and $93.98 for the last three months, and I got $22,728.99 Methodist Hospital bill, $1,168.25 Emergency  ambulance bill' $561.00 Arcadia Pathology bill, I need to keep this 2 evening job in master control room because it's my only way to get any income, the problem is that, after 7 pm, I will be working alone until midnight, only the fear of Disorientation attacking accompany me. I try to make myself very relieve on work, especially that I have to work alone, the regular sign out time is supposed to be 1 AM, but I always signed out at 3 AM, they found out and complained that my part time shift was slow than other full time people, also cost them 2 more hour over time to pay me, I don't dare to tell them that I need to deal with my Disorientation attacking fear and the headache hangover in my temples inside my head, not even my pinching pains in my lumbar' groin and flank. I already agree to sign out at 1 AM anyway, doesn't matter when can I finish my job.

The PNS pains in my body (I got 3 times job injured 1994' 2000 and 2002, while I was working for you and Sing Young Music World) deteriorated and attacked me.

Sunday, 3/9/08, I had attacked by Disorientation. The city of Pasadena Paramedics, (100 N. Garfield Ave. #121, Pasadena, CA 91109, 626-744-4179. IV Supplies $50. Supplies $25. ALS Emergency Base Rate $954. Mileage, 5= $17  Oxygen $54.25, Total= $1,168.25) sent me from 465 N Halstead, to Arcadia, Methodist Hospital on ALS Emergency.

I didn't have ALS attacked, According to the 24 hours staff report, I was 'awake and CONFUSED', could not say my name, kept asking where I was, kept saying I could not find my car. (I was so lucky, it did not attack me when I was driving???) They plunked my finger to get my blood for test, they use the lights on my pupil, but I didn't have any response to these things, and don't remember any of these.

Methodist Hospital kept me hospitalized' monitor watching and test everything for 4 nights. (3/9-3/13 Post Icu $7,500.  Cardiology $2,900. Mri -Brain $2,800. Laboratory $2,692. Ct Scan/head $1,800. Peri Vascul Lab $1,250. Stress Test $1,100. Emerg Room $850. Pharmacy $629.94. Eeg $600. Ekg/ecg $300. Dx X-ray/chest $280. Med-sur Supply $24. Sterile Supply $3, TOTAL CHARGES $22,728.99 )

I requested for Disable case and the hearing is at 4/14/2006.

Whatever happen after the hearing, with the documents from my hearing court, I will call for another lawsuit to u, to sue you and Sing Young Music for No medical insurance' Maltreatment' Harassment' Mail Fraud' Piracy to the employee.

Please settle everything with me before I'm calling this lawsuit. If you don't, my singYoungEtc.etc web sites will be always there. If you change your business name into any other name, those web sites will always change the domain name follow-up and according to yours.
